One of the things I carry in my doula bag to every birth is a TENS unit. This is a small machine that delivers an electrical stimulation to pads that are stuck to a woman's back. This stimulation reduces pain for most women that use it, and provides a good alternative to someone who is about to turn to pharmological pain relief in labor. Since drugs have side effects and the TENS unit doesn't, it is certainly worth a try. While most women won't want to bother with the wires, it can be very helpful for someone with back labor.
